Summary: We want to provide equipment such as raised toilet seats, hold bars etc., transportation, medicine, and rehab equiment to seniors who have limited insurance and resources.

Our vision is to help provide optimal health for the underserved elderly in the Bronx by providing vital health services and essential equipment to help with safety, functional status and independence(such as raised toilet seats, reachers) provide medication on an emergency basis not covered by insurance in order to provide better outcomes by avoiding rehospitalization, offering short term transportation to rehabilitation centers and critical doctors appointments, co-payments for medicine so that patients can transition smoothly back to their home after acute hospitalization. This project should take between 10-12 mos. or sooner. Our Club members will actively monitor and review those patients receiving the equipment and services.
