Project Description:

(1)What needs have been identified: The Rendezvous Senior Day Services is a non-profit adult day center that provides a place for seniors and people with disabilities to come to Monday - Friday to participate in activities, socialize, have lunch and build relationships. Members are invited to a variety of activities in the Center as well as outings in the community several times per week. Members have lunch together, play games, watch movies, participate in informative lectures and talks, celebrate birthdays and holidays, enjoy a variety of music and entertainment and have a place so they are not isolated or alone. Members may attend five days or as many days per week as they wish. The Center allows caregivers an opportunity to have scheduled time alone while tier loved one is cared for in a safe environment.

(2)How the project will meet those needs: The Rendezvous serves breakfast and lunch Monday through Friday. We are requesting financial help to install non-porous, cleanable back splashes and walls in our kitchen. We have asked a local contractor a price for the materials and installation, which would include extending a short wall in the kitchen. The estimate is $3500. This project would greatly enhance the kitchen and the ability to keep it exceptionally clean for the seniors who attend and eat at our facility.

Participants in the Rendezvous Senior Day Service Center are referred to as "Members". This is a community based group program designed to meet the needs of adults with disabilities and require assistance and supervision who:

>Have few or inadequate support systems

>Require assistance with activities of daily living

>persons with physical problems that require monitoring and supervision on a regular basis

>persons with emotional problems that interfere with one's ability to cope on a daily basis

>persons with memory loss and cognitive impairments

> person who require assistance in overcoming the isolation associated with functional

limitations or disabilities

>persons whose family and/or caregiver needs respite

>persons recently discharged from hospitals or nursing homes that need supervision daily

>persons who, without intervention, are at risk of premature institutionalization due to

physical deterioration or their psychological condition

>persons who need support in making the transition from independent living to group

care or in making the transition from group care to independent living

Persons meeting the above criteria are asked to fill out an extensive application with their caregiver or themselves. Rendezvous works with any other services the applicant may currently have a relationship with to make a plan of care and build goals for the individual while they are at the Center. These goals may include additional socialization, physical activities, nutritional improvement or a wide variety of issues. Members and their families may choose the amount of time to participate in the Center.

Currently, the Rendezvous daily census for activities and meals ranges from 12 to 20 depending on the day of the week. Through the year, as personal circumstances change (people move, needs change, etc) the Rendezvous Center will work with 60-75 individuals for varying lengths of time. Members may be with us for a few weeks, a few months or some have been attending for years.

For our participants, the Rendezvous Day Center's benefits are extensive:

• A safe, secure environment in which to spend the day

• Enjoyable and educational activities

• Improvement in mental and physical health

• Enhanced or maintained level of independence

• Socialization and peer support

• Nutritious meals and snacks

However, it takes a tremendous amount of Staff time soliciting seniors and their families. Identifying an individual in need of services, developing a network of resources, then utilizing the available services can be a lengthy process. While doctors and community health offices encourage seniors to participate in social settings, many individuals are reluctant to leave their homes. Outreach to this population often requires home visits, and numerous personal contacts before the individual actually takes advantage of the services at the Day Center. The personnel and wages needed to establish a relationship of trust with individuals can be quite costly.

The Rendezvous Center offers a medical equipment loan closet for the community. The wheelchairs, transfer chairs, rollators, transfer benches, walkers, commodes and other adaptive equipment relieves community members of costly purchases that often, are needed only temporarily. This aspect of the Rendezvous Center is free and available to all. In the past year, Rendezvous made 289 equipment loans to persons in our community.

Extremely important to the health and well-being of our Members is our food and nutrition program. As part of the Child and Adult Certified Food Program, the Rendezvous Center provides a healthy breakfast, lunch and snack to all participants. Malnutrition in the most vulnerable population groups - the young and the old - is a problem of increasing national concern. Recent studies have shown that one out of four older Americans suffer from poor nutrition.

Good nutrition plays a crucial role in keeping older people healthy and functioning. Many older Americans, however, aren't eating well. Those who may have lost their teeth find it difficult to chew. Others may have a hard time getting to the grocery store, especially if they no longer drive. Still others simply cannot afford to buy the kind of food that could help keep them healthy. In fact, for many older people it's not a question of eating well but of eating at all. Even today, millions of older Americans are hungry or worried about where their next meal is coming from.

The goal of the Rendezvous Nutrition Program is to provide nutritionally balanced meals to those who are 60 years of age or older. With the high cost of utilities, prescriptions, and medical needs while on a fixed income, senior citizens sometimes face the agonizing choice of either paying the necessary living items or purchasing food. When many seniors are already at an increased health risk, nutrition is of the utmost concern to help reduce colds and illnesses. The meals furnished through the Rendezvous Nutrition Program provide one-third the daily recommended nutritional requirements.

The Rendezvous Nutrition Program follows the guidelines of the adult and child federal food program. These guidelines require us to fulfill the healthy eating food pyramid of fruits, vegetables, whole grains, dairy and fish, poultry and meats. Meals and snacks are prepared with the utmost care for sanitation for this vulnerable population. But they must also be tasty, colorful and appetizing! Our Staff uses the USDA food program recipes as well as a variety of cookbooks and personal food ideas. We constantly survey our Members for their input on the quality, variety and appeal of our food.

The kitchen was remodeled a few years ago with a generous donation from Credit Union One and equipment purchased in 2013 from First Bank. The community of Ketchikan has been very kind to the Rendezvous Center. We are again asking for assistance from Rotary to install non-porous panels to the walls and back splash of the sinks and counter tops in the kitchen. Sanitation is critical in any food service operation, of course but more so dealing with the fragile immune systems of an older population. We have discussed the possibility of this project with Jerry Rivera, owner of F&R Enterprises, which is a local construction and remodeling company. Mr. Rivera has worked with Rendezvous on a number of projects including the previous kitchen remodel. His estimate for materials and installation for this particular project to be $3,000 to $3500.

As a non-profit organization, Rendezvous operates on a shoestring budget. We have channeled funding into the activities and Members of the Center rather than equipment in the kitchen. All of our pots, pans and small appliances have come from donations at the Thrift Store. We are also asking for $1,000 to purchase adequate culinary grade pots, pans, knives and small wares for better food preparation for our Members.

(3)What is the timeline for your project. Please specific: This project will be done within 120 days of raising the funds

(4)What will the project funds will be used for: With the new wall surface material in the kitchen that provides breakfast and lunch for the Rendezvous Senior Day Services, the caregivers will be able to keep the kitchen cleaner for years to come.

(5)How club members will be involved. The 1st City Rotary Club will contribute an amount equal to or more than the District grant to arrive at $3,500 for this project. The Club is prepared to provide at least $1,750.

Additionally, Licha Kelley-Smith teh Manager says: "I am very confident this project can be completed for the requested $3500. I have asked the advice of a local contractor for pricing. However, if the date of completion is 120 (4 months)days from approval, I know a returning Rotarian who is a master remodeler who will head up this project and get it done with no labor costs. He would need help from other club members moving furniture & materials and assistance with installation. Bruce is positive he can complete the project on budget. He has done several Rotary projects and is happy to be involved again. Jim Dahl can vouch for his abilities and thoroughness.

Absolutely, hang a plaque or something noting Rotary's generousity! We can also have a press release and pictures in the paper, on the radio and facebook."


Addendum to Project Description (added after the project was signed)

Per Chris Baca on February 10, 2015, the budgeted amount is actually $2,500 from $3,500 as originally stated. The club is still matching 100% of the DDF.
