Phoenix Allies for Community Health is a volunteer-led ,patient centered health clinic serving the medically marginalized by providing free health care with respect and cultural humility. Currently the Phoenix Allies for Community Health(PACH)clinic focuses is on disease prevention, primarilary chronic issues of diabetes, heart disease and high cholesterol. The PACH clinic sees the uninsured diverse population in west Phoenix area. The case load is 325 families, this is based on funding. If they had more funds, they could see more patients.
In addition to providing medical care to their patients, the volunteers include medical nutritional therapy, support groups, yoga classes and English classes. The staff provide cooking classes, how to garden classes and several health educational seminars.
Another function of PACH is to provide clinical experience for students pursuing careers in various healthcare professions such as nurses and Physician Assistants. The student volunteers are an integral part of the free clinic, which in turn gives them valuable opportunities to learn best practices in serving the underserved. This community collaboration is important and effective in keeping the cost of services down. Yet they still have healthcare services they would like to provide to their patients.
The PACH Clinic believes in providing high quality healthcare to all patients and this includes obtaining an electrocardiogram on every patient suffering from chronic diseases such as diabetes, hypertension, high cholesterol or thyroid disease. Early detection of heart disease is the best way to prevent heart attacks. The current EKG machine is an older used device that was donated and it's currently malfunctioning and needs replacement. We have identified the Nastiff CardioResting ECG System that will met the needs of the PACH clinic.
Many of the patients at the PACH Clinic are suffering from diabetes and part of the treatment plan is provide eye exams on each patient annually, or more often if diabetic eye disease is detected. This is a vital component to prevention of diabetes-caused blindness. A slit lamp not only identifies diabetic retinopathy, but offers us a chance to check for other eye diseases and eye problems for their patients free of charge. We have identified 2 x 3 Step Haag streit Slit with camera and surgical motorized table that will met the needs of the clinic.
We are requesting funds in the amount of $3,264 for the ECG system and the Streit Slit lamp with camera.
The North Phoenix Rotary Club would like to provide 2 hygiene drives to support the PACH clinic, to include toothbrushes, toothpaste, floss and other necessary products.
Addendum to Project Description (added after the project was signed)
The PACH clinic found another source for ECG system and Streit Slit lamp because delays in Rotary District Grant funding. PACH clinic had a need for Vital Signs Monitor which cost $2,744.77 including shipping. North Phoenix Rotary agreed to fund this monitor as it served the same health care organization and patient group. North Phoenix funded $2,656 of which $1,140 were from the district grant monies.
