Durante o ano rotário de 2020-21 nós estávamos realizando análises de necessidade para a realização de um projeto global em nossa região. Ficou evidente que o HUM (Hospital Universitário de Maringá), por ser uma unidade diretamente impactada pelo COVID19, se viu em situação delicada com relação à manutenção dos seus equipamentos da UTI Neo-natal.

Neste projeto, a instituição a ser beneficiada é o Hospital Universitário de Maringá - HUM, um hospital público que beneficia toda e qualquer família que necessita dos serviços deles, de forma gratuita. A unidade diretamente beneficiada deste projeto é a UTI Neonatal deste hospital. Este projeto trará benefício direto à Recém nascidos que necessitam de atenção especial. O estudo de abrangência do Hospital Universitário de Maringá revela que são 2 milhões de habitantes que podem, eventualmente utilizar os serviços do HUM.

De abril a junho realizamos estudos para levantamento do cenário de cuidados neonatais na região metropolitana de Maringá e em conjunto com o HUM pudemos detalhar ainda melhor estes números.

Após breves conversas realizadas com profissionais da saúde, realizamos pesquisa dos dados de hospitais próximos para compreender a volumetria e capacidade dos hospitais quanto ao atendimento neonatal.

Posteriormente, ao realizar reunião presencial com o corpo técnico do HUM, foi informado que as incubadoras haviam sido descontinuadas, e portanto eles não conseguiam mais realizar compra de peças para conserto. Isso nos chamou a atenção e solicitamos inventário de incubadoras e orçamento de fornecedor. Com os dados em mãos, pudemos definir as necessidades que poderiam ser atendidas por um projeto Global da Área de Enfoque de Saúde Mães e Filhos:

Necessidades identificadas que o projeto pode atender:

1. Aumentar o número de atendimentos da UTI Neonatal do HUM

2. Melhorar a qualidade do atendimento da UTI Neonatal do HUM

3. Permitir uma redução na mortalidade infantil da região por meio da melhora dos atendimentos da UTI Neonatal do HUM

Nós identificamos que não seríamos capazes de realizar a troca de todas as incubadoras, por ser financeiramente um valor muito alto, portanto este objetivo foi removido do escopo do projeto.

Como pontos fortes do contexto do projeto, identificamos que, felizmente, temos fornecedor com potencial de atender as necessidades do projeto, devidamente qualificado para tal. Portanto, o objetivo foi definido para atender as necessidades listadas, aproveitando os pontos fortes e fazendo com que, mesmo com necessidades listadas que não podem ser atendidas pelo nosso projeto, que conseguiremos melhorar muito o atendimento desta unidade.

A atividade proposta deste projeto é a realização de compra de novas incubadoras pois, infelizmente, os recursos repassados à unidade Neonatal do Hospital Universitário de Maringá não são suficientes para realizar tal melhoria, já que neste momento há muito esforço direcionado ao Covid.

Atualmente, as incubadoras descontinuadas à 14 anos continuam em operação, e o número de leitos disponíveis vai reduzindo conforme estas máquinas deixam de funcionar ou são utilizadas como fonte de peças de reposição para outras máquinas.

Nosso projeto, portanto, foi definido pela renovação destes equipamentos. Com o orçamento realizado, em alinhamento com a chefe da UTI Neonatal, foi entendido que uma solução viável e impactante seria a realização da compra de:

• 3 unidades da Incubadora para Recém-Nascidos 1186C

• 1 unidade da Incubadora Híbrida de Cuidado Intensivo Neonatal - Duetto 2386

• 1 unidade do Berço Aquecido Ampla 2085

During Rotary year 2020-21, we were conducting a needs analysis for a global project in our region. It was evident that the HUM (University Hospital of Maringá), as a unit directly impacted by COVID19, found itself in a delicate situation regarding the maintenance of its equipment in the Neonatal ICU.

In this project, the institution to be benefited is the University Hospital of Maringá - HUM, a public hospital that benefits any family that needs their services, free of charge. The unit directly benefiting from this project is the Neonatal ICU of this hospital. This project will directly benefit newborns who need special attention. The comprehensive study of the University Hospital of Maringá reveals that there are 2 million inhabitants who may eventually use the services of the HUM.

From April to June, we carried out studies to survey the neonatal care scenario in the metropolitan region of Maringá and, together with the HUM, we were able to detail these numbers even better.

After brief conversations with health professionals, we conducted an analysis of data from nearby hospitals to understand the volume and capacity of hospitals in terms of neonatal care.

Later, when holding a face-to-face meeting with HUM's technical staff, it was informed that the Neonatal ICU incubators had been discontinued, and therefore they were no longer able to purchase parts for repairs. This caught our attention and we requested an incubator inventory and supplier quote. With the data in hand, we were able to define the needs that could be met by a Global Project of the Maternal and Child Health Focus Area:

Identified needs that the project can meet:

1. Increase the number of newborn that receive medical care at the HUM Neonatal ICU

2. Improve the quality of care at the HUM Neonatal ICU

3. Allow a reduction in infant mortality in the region through improved care at the HUM Neonatal ICU

We identified that we would not be able to make the exchange of all incubator units, as it is financially a very high value, so it was removed from the scope of the project.

As strengths of the project context, we identified that, fortunately, we have a supplier with the potential to meet the project's needs, duly qualified to do so. Therefore, the objective was defined to meet the listed needs, taking advantage of the strengths and ensuring that, even with listed needs that cannot be met by our project, we will be able to greatly improve this unit's service.

The proposed activity of this project is the purchase of new incubators because, unfortunately, the resources transferred to the Neonatal Unit of the University Hospital of Maringá are not enough to make such an improvement, since at this moment there is a lot of effort directed to Covid.

Currently, incubators that have been discontinued for 14 years are still in operation, and the number of available beds decreases as these machines stop working or are used as a source of spare parts for other machines.

Our project, therefore, was defined by the renovation of these equipments. With the budget made, in alignment with the head of the Neonatal ICU, it was understood that a viable and impactful solution would be to purchase:

• 3 units of the 1186C Newborn Incubator

• 1 unit of the Neonatal Intensive Care Hybrid Incubator - Duetto 2386

• 1 unit of the 2085 Ample Heated Cradle
