Preamble

Rotary Club of Pune Central with the Global Grant will aid in establishing an Aphaeresis Technique at JANAKALYAN RAKTAPEDHI, PUNE, which has a well-equipped component laboratory for the last 14 years. The Aphaeresis Technique is a modern technique for component separation utilized for better patient care. The requirement of blood component prepared by this technique is increasing in Pune for following type of treatments:

1. Haematology oncology and other speciality services -

Haematology, Oncology and other super speciality services are now available in Pune. These services require platelets prepared by Aphaeresis technique for better patient care particularly patients suffering from Dengue.

2. Dengue fever -

In the Last few years it is observed that incidence of 'Dengue 'fever or 'Dengue like 'fever has increased. These patients have low level of platelets and therefore require platelet concentrate transfusion.

3. Numerous diseases dealt by all faculties nowadays require this aphaeresis facility for better patient care.

Objective of Aphaeresis Project:-

Modern technique of aphaeresis should be made available to needy patients for better patient care which will lead to increased patients survival with participation from everybody i.e. society, blood bank and the patient/donor.

Working of Aphaeresis Technique: - Under the supervision of trained technical staff online blood is withdrawn from the donor into the machine. Machine then separates required constituent of blood and re-infuses the remaining blood in to the body of donor.

The separated constituent i.e.is blood component is stored in special storages and transfused to patient whenever required.

A) Cytapheresis (Cell separation):- Red blood cells, White blood cells (Granulocytes), Platelets, Peripheral blood stem cells etc are separated from blood.

B) Plasmaphaeresis:- Plasma is separated from blood of donor. This procedure is also done to treat patient (Therapeutic Plasma aphaeresis)

The time required for Aphaeresis Procedure is one to two hours.

Blood Components Prepared by Aphaeresis Technique are: Single Donor platelet, Plasma, Fresh Frozen Plasma, Concentrated Red Blood cells, Buffy coat depleted red blood cell concentrate, Granulocyte concentrate & Peripheral blood stem cell

General Requirements for Aphaeresis

• Area- 10 square meters, well lighted and air conditioned.

• Donor bed and emergency trey to tackle any reaction to donor

• Trained technical staff - Medical Officer, Nurse, and Physician

Janakalyan Blood Bank, Pune has spacious and well equipped component storage area for proper storage/ preservation of components prepared from aphaeresis procedure/ technique. Well-Knit team of trained Pathologist, Medical Officers, Blood Bank Technicians and Nurse are available round the clock. Team of Honorary Consultants from other faculties like Medicine, Surgery Anaesthesia are also available. This trained technical manpower is critical for aphaeresis procedure.

Other facilities like blood grouping and compatibility testing are also available.

Advantages of Aphaeresis Technique:-

1) Only required constituent of blood is removed from the donor and remaining constituents of blood are rein fused, hence it is beneficial for donor.

2) Since testing for HIV, Hepatitis B and C, malaria, syphilis, etc., is done before the procedure, of discarding of the component is ruled out.

3) Some advantages of Aphaeresis are described by comparing platelet concentrate

4) This Technique is primarily used for taking out platelets from blood donor which are 5 to 6 times effective over conventional Platelet Concentrate.

5) It also avoids infection of platelet refractination & Transfusion Transmitted Diseases

Financials -

Cost of Aphaeresis Machine = Rs 15 lakh or $ 25,000

Cost of Disposable Aphaeresis kits = Rs 9 Lakhs or $ 15,000 for 150 kits free for needy patients

Total = Rs 24 lakhs or $ 40,000

Who will avail services of Aphaeresis techniques from Janakalayan Raktapedhi, Pune: Janakalayan Raktapedhi is working with the Regional Blood Transfusion Council for a long time. About 100+ Hospital are taking benefit of blood service. Further they also work through a network of 12 blood storage centres spread in Pune City, Pune and Satara Districts, which are in a radius of about 100 km from Pune city. Blood and blood components to medium sized hospitals situated in these areas are catered by these centres. These hospitals are in need of Aphaeresis technique support so that they can provide better, effective, and timely health service.

150 kits will be available every year for free distribution to Rotarians. With a ten year life span, 1500 free kits will be distributed within this period and around 5000 patients will benefit.
