Proposal Form

An organization in Brazil provides therapeutic services aimed at improving motor skills of children and adults suffering with cerebral palsy, down syndrome, hearing or vision impairment and mental disabilities moving them towards independence. The equipment will be used by existing staff. The needs of the community have outpaced the organization's funding and patients needing treatment are unable to receive care due to the lack of devices. Once approved, and the timeline for funding is determined, competitive bids will be obtained and equipment ordered and put in place. The partner Rotary Club in Brazil will interview supervisors at the facility regarding expected results from use of equipment and will check weekly to see that staff is maintaining logs recording patient use of equipment. After a three month period of use, club members will again interview supervisors to determine if use of the equipment has beneficially aided the organization's patients.

Funding for monies needed by Escondido Sunrise Rotary has been obtained from non-Rotarian sources. Funding by our Brazilian Rotary partner consists of DDF already earmarked for the project. No DDF is requested from District 5340. No additional funding appears to be needed.

One purpose for the grant is to demonstrate to a non-Rotarian who has established a multi-million dollar bequest to support similar projects in Brazil and in the United States that The Rotary Foundation does provide the opportunity to support the good work which can be done by funds he places in an endowment.

From Susan Doxtator at TRF:

The last information I received from João Carlos Cazú of RC São Carlos was in May 2015. The host club had the equivalent of a little over US$4,000 left over after all the purchases were made because of some price reductions and exchange rate variations between the US$ and the Brazilian real.

They requested to make additional purchases of physical therapy equipment and I approved the request changes in June.

If you want to get a status update from the sponsors, I would say to contact João Cazú and PDG Guilherme Cleber Marconi (callname = Cleber). They probably are pretty close to being able to start working on the final report for GG1417315, but they would be able to confirm that.

Susan Doxtator at TRF advised today:

João Cazú from RC São Carlos started a final report for GG1417315 but not all of the information is entered yet. You might start by asking him for a progress update on the GG1417315 so that you can update your club.

Note that the report isn’t due until 4 March 2016 at the latest (12 months from payment date), so getting a report entered and submitted to TRF isn’t an urgent matter until next year.
