Name of the project: "Healthy daughter today - Happy mother tomorrow"

Project Goals:

To improve access to essential medical service and support trained health care providers for mothers and children.

To promote equal opportunities for all people in region in spite of having one Department for all Balkan countries.

To promote effective advanced surgical operation as ways of solving the problems comparing to old techniques.

To increase awareness of the problem with child and adolescent gynecology to the public and to the young persons with gynecological problem.

To increase quality and standard of living and health care in countries without possibility to open Department for child and adolescent gynecology because it is not cost effective for any hospital.

To conduct and encourage programs of medical education in the field of pediatric and adolescent gynecology.

To encourage and support professional training programs in pediatric and adolescent gynecology.

To promote communication and collaboration among health care professionals on issues related to pediatric and adolescent gynecology.

To provide the equipment needed for "Department for child and adolescent gynecology" in order to meet the goal.

Project Users: Child and adolescents from age 0-18

Proposed activities:

• Purchasing laparoscopic system

• Organizing "Promotional night", during charter ceremony.

• Holding a press conference

• Providing promotional material for further activities in the course of advocacy for maternal and child health (photos and short video)

Expected results of the project:

The equipment will enable doctors and health care professionals to engage their best knowledge for diagnostics and operations.

Better quality of the post operative process.

Sensitized public and media on the issue of pediatric and adolescent gynecological issues.

Created promotional material for further activities

Evaluation plan:

Program and Financial reports will provide information about the project activities which will be basis for evaluation.

Evaluation Committee will evaluate whether the results are to meet the goals.

EXPLANATION OF THE PROBLEM

This would be the first laparoscopic system that Department for child and adolescent gynecology would obtained. It would be first in Serbia, but also in region that one institute have.

Doctors had published several scientific papers on how to improve ultrasound diagnostics with utilization of new equipment.

They prepared scientific papers ready for publishing with subject related to utilization of laparoscopic operations in child surgery and importance of conservation of ovaries during operation time.

In the last seven years, hospital has been performing surgeries in children's surgery room with borrowed and worn out equipment. This equipment does not have features necessary to complete laparoscopic treatment.

We have operated on more than 250 girls, all younger than 20 years, and more than 260 tumors (37 of them had malignant biological behavior.)

Advantage of laparoscopic approach:

• Small cut on the skin

• Less invasive operation

• Faster postoperative recover from 4 to 7 times

• Shorter time needed to be spent in the hospital from 2 to 3 days comparing to classic operations, where average time spent in the hospital is from 7 to 10 days.

• With this system we are having significant less lose of the blood and need for transfusion.

• Less adhesion that appear after operation.

• Generally, better protection of reproductive potential of the patients.

New equipment with the new processor would allowed doctors, for first time in history of the Institute, to equip gynecology room and organize more frequent laparoscopic surgeries helping those in need.

This is especially important when tumors or cysts are twisted; In this case they would need equipped room with the laparoscopic technology to save the ovary. This type of complication is happening with third of their patients, which is more often than with adults.

This equipment would be used for other diagnostic procedures such as vaginoscopy and hysteroscopy, and would especially benefit younger patients. During the last period Institute have had 11 patients with malignant tumors in genital section and they all needed these procedures.

Advantage of this technology:

1. The processor can be connected to HDTV laparoscopes (10mm, 5mm), video arthroscopies, video cist scopes with flexible head, HDTV video laparo-toraco scopies with flexible head, video scopies with "single port" utilizations, tracheal video scopies. This is very important since this equipment is multidisciplinary and can be used by other doctors in hospital, such as orthopedics, urologists and anesthesiologist.

2. This processor has an option NBI (Narrow bend Imaging). This system allows fine vascular visualization and it emphasizes veins and capillaries. This is important because of endometriosis with adolescents that cause pain in abdomen and it is not possible to diagnostic without this processor.

3. Flexible monitor hand featuring 3d movements, capable of mounting on either side.

4. Insufflator UHI-3 has the automatic feature to extract smoke from the abdomen. This way operator would be protected from the toxic and cancerous matters. In combination with electrosurgical unit UES-40 automatic extraction of smoke during the first few cuts is ensured.

Maybe the most important goal of this project is to provide access to essential medical service for society, but it has to be achieved along with many other goals which are conditional for this goal.

These "lesser" goals that are to be met before, we can create the environment where families will be informed about importance of preventive health care, but also serene, if situation bring their child to hospital. Those goals are divided into long term and short term objectives:

Long term objectives:

• To have qualified doctors who will have opportunity to learn and progress with laparascopic methods.

• To introduce preventive health care as obligatory and standardized therapy to society.

• To have pediatric and adolescent gynecology introduced to the public and to the families with daughters.

Short term objectives:

• To have the quality and superior equipment improved as well with trained doctors, so that Department would be able to facilitate large number of the patient.

• To have the equipment needed for "Mother and child health care Institute of Serbia" in order to meet the other "higher" goal;

• Gaining trust and faith in medical system of Serbia;

• Raised self-confidence of families by achieving better results from Department and gained trust will bring closer doctors and Department to the patients and public.

• Development of cooperation with media;

• Large number of people in society involved into subject "pediatric and adolescent gynecology";

• Opportunity created for new friendships, team spirit, exchange of information and knowledge.

ACTIVITIES

Previous activities provided by Rotary Club Belgrade Metropolitan:

• Promotional activities :

- Preparing press releases and informing public about this sensitive topic - (daily newspapers, internet forums and social media, radio and TV)

• Equipment:

- Collecting of written offers from suppliers for laparascopic equipment,

- Talks with suppliers, selection and negotiation over terms of purchase,

- Negotiation over spare parts,

- Participating in Global grant project with Italian and Serbian rotary clubs providing DNA sequencer for "Mother and Child health care institute".

Planned activities by Rotary Club Belgrade Metropolitan:

• Preparation Activities :

- Preparing Global Grant application,

- Organization of Rotary club Belgrade Metropolitan charter ceremony with the purpose of involving other Rotary clubs in this project and presenting pediatric and adolescent gynecology and their difficulties to potential donors,

- Informing all Rotary clubs and friends of the Rotary club Belgrade Metropolitan about this project.

• Laparascopic equipment:

- Purchasing the laparoscopic equipment,

• Reception of purchased laparoscopic equipment

- Setting and testing of purchased laparoscopic equipment

• Promotional Game:

- Providing Media Coverage by organizing press conferences prior delivery of the equipment,

- Design and printing of promotional material (t-shirts, posters, pens, etc.)

• Final Report:

- Preparing an evaluation report (program and financial) on the project activities.

SUSTAINABILTY NEEDS AND FUTURE FINANCING

Purchasing equipment from local supplier Institute will be provided with long term warranty period, service and spare parts for next 10 years. Equipment will become property of the Institute and will enter to medical system of Republic of Serbia, therefore Government of Republic of Serbia will have responsibility to finance usage of laparascopic tools. With special contract between Rotary club Belgrade Metropolitan and Institute, user of the equipment will be obliged to maintain equipment according to specification of the producer.

Since this is the only department for pediatric and adolescent gynecology in region that includes Slovenia, Croatia, Serbia, Bosnia and Herzegovina, Montenegro, Macedonia, Romania and Bulgaria, we hope this department will become symbol of fight for healthy daughters but also future mothers.
