What are the main objectives of the project, and who will benefit from it? We care for those who suffer from kidney problems of any kind. With almost 2.5 lakh people dying of kidney failure in India every year, it is the third-largest killer. We aim to connect and support people undergoing dialysis and create awareness among the public of dialysis, continuous renal replacement therapy and other treatments. Rotary Club of Moradabad Sanskriti proposes to set up a dialysis centre at Moradabad, Uttar Pradesh in North India to offer highly subsidized dialysis treatment to the poor and downtrodden patients. The project envisages setting up a 5 bed dialysis centre at Moradabad. Which area of focus will this project support?

Humanitarian Projects - Project Planning Describe the community needs that your project will address. What is dialysis: Dialysis is a treatment that does some of the things done by healthy kidneys. It is needed when one's own kidneys can no longer take care of the body's needs. When one's kidneys fail, dialysis keeps the body in balance by: 1. removing waste, salt and extra water to prevent them from building up in the body 2. keeping a safe level of certain chemicals in the blood, such as potassium, sodium and bicarbonate 3. helping to control blood pressure Needs of the community: According to various studies, it is estimated that more than 150 people (per million) suffer from Chronic Kidney Disease (CKD) in India. This figure is growing at around 7% year on year. With diabetes also increasing quite alarmingly, CKD figures are expected to shoot-up in the years to come. One of the major life-saving treatments for CKD is dialysis which may he required to be done as frequently as once a day to twice a week in order for the patient to stay alive. A person on dialysis can even survive for up to 20 years, if done regularly. Unfortunately, dialysis centres in India are few in number and therefore are unable to meet the demands of the population. In addition, private hospitals and dialysis centres charge anywhere between Rs. 2000-Rs. 5000 per session, taking it out of the reach of the poor and even the middle-class population of India. How did your project team identify these needs. The project team performed two activities to identify the need. The first was a literature survey and the other was a community assessment study conducted in the Moradabad area. How were members of the benefiting community involved in finding solutions. The sample of the benefiting community were interviewed as part of the community assessment study. Will you work in coordination with any related initiative in the community? If yes, briefly describe the other initiatives and how they relate to this project. If no, please explain. Are local initiatives not addressing these needs? Or, if they are, why did you decide not to work with them? YES The government of India has set up a Dialysis Program under the National Health Mission and is looking for Public Private Partnership models to meet the growing demands for this life saving treatment. Please describe the training, community outreach, or educational programs this project will include. All hospitals, private clinics and doctors, especially nephrologists in the area will be intimated about the existence of the new facility and will be asked to forward CKD patients from the economically weaker sections to the dialysis centre. This will do through direct visits, telephone campaigns, voice drops, pamphlet distribution and large hoardings where possible. How were these needs identified? Since all dialysis treatments have to be point doctors, clinics and hospitals become the first contact point for patients with CKD. Creating awareness about the new dialysis unit among the medical professionals would be the best way to ensure full utilization of the dialysis centre. What incentives (for example, monetary compensation, awards, certification, or publicity), will you use, if any, to encourage community members to participate in the project? No monetary incentives will be given. This is a lifesaving treatment specifically for those diagnosed with CKD and hence the patient and their families require no incentive beyond guaranteed high-quality dialysis at highly subsidized rates. Providing incentives to medical professionals for referrals is a highly suspect method of garnering dialysis patients, to put it mildly. To those working at the dialysis centre, we would provide incentives by way of recognition during club functions, certificates of appreciation, etc. List any community members or community groups that will oversee the continuation of the project after grant-funded activities conclude. - Budget Will you purchase budget items from local vendors? Explain the process you used to select vendors. YES Information on various brands of dialysis machines available in the market have been collected. Also, opinions and suggestions have been collected from Rotary run hospitals and dialysis centres. Then quotations have been collected from the short-listed brands. The final decision has been taken based on multiple criteria including 1. Price, 2. Presence [No. of service personnel available in Moradabad, availability of a service level agreement, location and proximity of warehouses, provisions for replacement machines in case of major breakdowns, and over all track record of the company.] Did you use competitive bidding to select vendors? If no, please explain. - Please provide an operating and maintenance plan for the equipment or materials you purchased for this project. This plan should include who will operate and maintain the equipment and how they will be trained. - Describe how community members will maintain the equipment after grant-funded activities conclude. Will replacement parts be available? - If the grant will be used to purchase any equipment, will the equipment be culturally appropriate and conform to the community's technology standards?
