1. Describe the benefiting community including its location, using any relevant geographic and demographic information (Please be as specific as possible). If the activity is a scholarship, enter "N/A". Machala, a city of 260,000 in El Oro Province, Ecuador is located in the southwestern portion of the country and adjoins Peru. Machala is the largest city in the province and a referral point for healthcare in the region, included Peru. The closest large city, Guayaquil, is about 300km and 3 -4 hours by road transportation. The Teofilo Davila Hospital is the principal public facility in the region. The project proposes to provide specialized medical equipment for the Neonatal Intensive Care Unit. In 2011, there were 3,143 babies born at the hospital of which 1,492 were classified as high risk. (complicated pregnancy, caesarian, premature births, etc.) In the same year, 561 were admitted to the NICU. This number included 70 referred from other hospitals and cities. The mortality rate in the unit is 7.1 percent. With this volume of patients and their special needs, additional and new equipment is needed in two areas - Neonatal respirators to assist breathing and LED lighting for treatment of jaundice. In 2011, 169 newborns required respiratory help but only 84 could be treated with the others having to be transferred to Guayaquil. With the addition of an third respiratory unit, 50% of the need will met.
In the same year, 350 babies required light therapy. Only 44 could be treated. With the addition of five LED light therapy units all babies requiring this service will accommodated locally. The hospital will provide the professional nursing staff to operated the equipment under the direction of four neonatologists. Maintenance and servicing will be provided by the hospital and supplier. In summary, the grant provides for one Newborn Respiratory unit at US$ 27,972 and five LED Therapy Lamps at US$ 2,240 for a total cost of US$ 39,172 With the approval of the project by October 31, 2013 the project can be completed by February 28, 2014. 2. What community needs have been identified? If the activity is a scholarship, enter "N/A". l. Purchase and install some necessary hospital medical equipment in the area of newborns requiring intensive care or are in medical emergency situation. Specifically a machine breathing and five LED lamps. Breathing machine to help newborns who can not breathe on their own, due to his poor development at birth, and LED lamps will decrease bilirubin pigment that affects skin and all its organs. Both conditions can cause severe damage, even death. 2. Provide effective training methods for professional doctors and hospital staff to learn to properly use equipment acquired through this project in order to instruct them in effectively treating critical conditions in which newborns are, and to make certain that its use will be made correctly. 3. Communicating through appropriate channels in Machala community about the improvements made in the Hospital, ensuring that this information reaches the greatest number of people in Machala. 4. Spread information on prenatal care, which should deter pregnant women to avoid the possibility of their newborns being in a position of risk and require special medical emergency treatments. 5. Through the spreading of information, seek to reduce the incidence of serious illness and death in newborns. 3. The hospital currently has two neonatal respirators and actually needs two more to fully meet the demands of the local and referral area. This project will provide one of these respirators. The hospital has available two LED Light therapy lamps. This project will provide an additional five units to meet requirements. 1. The community hospital, being publically owned and operated will act as a principal local contact for the community. The Rotary Club, also being a community organization, acts as a community contact. 4. Has the benefiting community(ies) confirmed that it would like the activity(ies) to take place? The community through their public hospital have entrusted the hospital with the responsibility of obtaining specialty equipment for the treatment of newborn infants. 5. 2. Please describe how the activity(ies) will address the goal(s) of the area(s) or focus. The goals of the project are to improve the health of mothers and their children by: a) Reducing the mortality and morbidity rate for newborn infants in the community served by the hospital. b)Improving access to essential medical services and health care providers for mothers and their children. c)providing prenatal care educational activities. 1. What will be the immediate and long-term outcomes of the activity(ies). A decrease in the mortality and morbidity rate of newborns. Eliminate the necessity of total blood transfusions for bilirubin effected newborns. Greater utilization of the NICU in the hospital Education of women on the necessity of obtaining prenatal care. Fewer transfer of newborns to more specialized services in Guayaquil. 2. Explain how all involved parties will act to ensure the sustainability of the activity(ies) and/or outcomes. The Hospital is committed to maintaining medical equipment and provide training as needed to ensure the use of long-term equipment and during its operational life. The Hospital will provide the necessary security to protect equipment within the scope of its comprehensive plan for protection of the equipment The RC of Machala Moderno will oversee the continued operation of the new teams in the NICU. The reports will be presented at club meetings on ongoing operations. The hospital will encourage and support visits of Rotarians in the hospital for assure a mutual relationship, effective, long term. The host sponsor, with support from the Hospital, will launch the educational outreach program for one year after the start of the project. The Hospital will perform the program thereafter, in collaboration with area physicians, other community groups and the health ministry.
