Summary: COVID 19 materials needed by Good Shepherd Manor & Center of Hope Food Pantry support.
Bradley Bourbonnais Rotary will donate $1000 for COVID related materials, including protective nursing home equipment, to serve the 110 developmentally challenged men residing at Good Shepherd Manor in Momence, IL.; plus donate $575 to the Center of Hope food pantry for community food support during the COVID disaster.
Addendum to Project Description (added after the project was signed)
Rotarians received, reviewed, and agreed with the needs assessment (attached) provided by Development Director Erin Richey from Good Shepherd Manor, and then wrote the necessary checks to both Good Shepherd Manor and Center of Hope.

Hello,
I want to remind you that your club's District Grant Final Report was due 30 days after completion of the project. COVID-19 grants were due by November 30, 2020. To complete your report, log into your account on matchinggrants.org. You will find the final report form there. Please complete the report and upload it along with your receipts by January 31, 2021.

Due to covid-19, meeting in person was obviously not allowed or appropriate. Therefore, in answer to final report questions 2 and 3, the number of Rotarians involved and subsequent actions they took should read as follows: Rotarians received, reviewed, and agreed with the needs assessment (attached) provided by Development Director Erin Richey from Good Shepherd Manor, and then wrote the necessary checks to both Good Shepherd Manor and Center of Hope. Based on the above notations, Stewardship review is complete. Meets all necessary criteria. Final report reviewed. Project okay to be approved by District Approval Committee. WL/LG
