Appeal for participating in Global Grant -- ESTABLISHING PEDIATRIC NEUROLOGY UNIT

About our RC Pune Heritage -- Our Club, Rotary Club of Pune Heritage, is two years old club. We have sixty active members with variety of classification. We have few experienced rotary members as well as new enthusiastic young rotary members. In last two year we have successfully carried out more than 100 community service projects in six focus areas of Rotary. Our project of upgrading Pediatric Intensive Care unit at Sassoon Hospital Pune done last year has provided most needed services (at almost zero cost) to more than 3000 children and saved many lives. With this good experience this year we decided to undertake this global grant project with Sassoon Hospital.

About Sassoon Hospital -- Sassoon General Hospital is a large tertiary care referral centre in Pune, with 1296 beds. It is a run by the Government of Maharashtra. The hospital serves, almost free of cost, the needs of patients from urban and rural Pune. Other Rotary clubs in Dist. 3131 also have done many medical projects in association with Sassoon hospital. Global grant project of "Donation of van for collection of human milk" has saved many lives.

About Project -- Establishing Pediatric Neurology Unit (Focus Area - Disease Prevention & Treatment)

Presently, Pediatric department of Sassoon Hospital attends nearly 1500 to 2000 admissions per year for neurological disease treatment. Such patients require specialized investigations/tests to diagnose the disease at early stages, so that correct treatment to the patient can be given in time. Presently there is no Government facility in Maharashtra for Pediatric Neurological investigations like Video EEG, EMG/NCV, VEP etc. Such investigations are very expensive in private hospitals and are out of reach for poor patients. In view of our earlier year association, hospital authorities have approached & requested our club for helping them to establish Pediatric Neurology Unit in Sassoon hospital.

Total cost approximately USD 40900.

BENEFICIARY

The donated equipment will be used for around 1500 to 2000 children patients admitted to Pediatric ward. These patients will treated free of charge. Cost of such treatment in private hospital may range from INR 15000 to 20000 (approx.. USD 230 to 300). The diseases like, Developmental delay, Cerebral Palsy, Epilepsy, Neurodegenerative disorders, Vision and Hearing problems, Neuromuscular diseases like Duchenne Muscular Dystrophy, Behavioral disorders like Autism/ADHD will be investigated with the donated equipment.

We have already raised our own club contribution and few local clubs are also ready to contribute. District 3131 leadership has given in-principle approval for USD 300 DDF. To make the project work as above, we are requesting USD 10000 DDF / contribution from your district / club.

Dear Sir,

We have explained all your points as under.

1. Since it is a Public and private partnership for this hospital, we need to have a solid commitment from this hospital administrators / state health department that all equipment provided from this grant will be used for needy patients. Also we prefer to have Rotarian committee to supervise the utilization as there is a possibility of abuse and corruption.

We appreciate and fully acknowledge the concern of donors. The government is committed to take care of the donated equipment and annual maintenance/repairs cost is fully borne by the government. Sassoon caters to low income and mid income group patients who are needy. We are successfully running a 60 bedded tertiary level (III B) Neonatal ICU which was funded through PPP. We have also used PPP to fund the Pediatric ICU (11 bedded level III), Human Milk Bank and a human milk van. All these projects are being successfully managed and have immensely benefited the poor and needy patients. For the NICU hospital has an oversight committee comprising the Dean, Medical superintendent, HoD pediatrics, In-charge of NICU and representatives from the donor.

The committee meets every month to discuss and review functioning of NICU. The total project cost of NICU is approx.14 crore rupees ( USD 2170000).

Hospital has started Maharashtra University of Health Sciences (MUHS) approved fellowship courses in NICU and PICU .

If this project (Pediatric Neurology Unit) is successful, hospital intend to start Pediatric Neurology fellowship as well.

Additionally our club will sign detailed MOU with the hospital regarding working, maintenance & reporting about the donated equipment. Our club is having a separate committee to supervise & monitor this activity.

2. Is there is any chance that affordable patients can pay nominal fee for these special tests and help in ongoing cost.

Patients who are below poverty line are not charged, however close to 30 percent of our patients are above poverty line and they pay highly subsidized charges for the services (e.g. X-ray is charged at Rs 50 approx.1 USD). Similarly these special tests will also be charged but at a much lesser rate than private hospitals (e.g. CT scan that costs 3000 rupees in private hospitals is being done for 800 rupees at Sassoon for patient above poverty line).Our club committee will monitor this activity, collect the data of patients treated and how much they are charged.

Last year our club had donated few equipment for pediatric ICU and one dialysis machine to the same hospital. We have got very satisfactory reports & records about equipment usage.

3. Please make sure that there is a service warranty for at least 5 years

Our club is going to sign MOU with the hospital and onus lies with the Government to undertake AMC /CMC of the equipment donated once warranty period is over. As a part of MOU, our club committee will monitor this activity.

4. Your club can do ongoing survey by using social workers for utilization

Our club will undertake separate camphene project under PI to make aware the community in & around Pune so that needy people will come to know & will use the donated facilities. Our club will see that all rural hospitals in Pune district will come to know about such facility.

5. Let us know if your club is involved in district and /or GG in the past and from your notes your club is involved in community projects.

Our club is a new club and has not yet participated in any global grant / district grant project. However, our club have about nine senior experienced Rotarians who can actively participate in execution of global grant project.

6. We wanted to make sure your district is committed for their contribution

Our club has already approached to our district global grant committee for DDF.

7. How are the technicians are trained for these special equipment

Sassoon hospital has a technician who is trained for pediatric EEG and has done about 1800 EEGs in children. They intend to train him for EMG/NCV/VEP/BERA with the help of Pediatric neurologist Dr Umesh Kalane who is affiliated with Sassoon hospital since 2010.

8. What contribution the Government hospital is doing / special rooms for equipment etc. , power source/ generators if needed

Government / hospital authorities have committed to provide space, electricity with generator backup, water, consumables, support staff, technicians, doctors etc required for this project.

9. Is this a General hospital with pediatric specialty care

Yes. Sassoon hospital is a 1500 bedded hospital with 120 general pediatric beds, 60 neonatal intensive care beds and 11 bedded PICU. We also have 6 bedded Thalassemia Day care unit and various specialty services like Epilepsy OPD, TB OPD, New born follow up OPD etc.

10. Do they have a Neonatology dept .

Yes, the hospital has a level IIIB NICU with 12 ventilators (including 2 HFOV), 8 bubble CPAP, TPN, Portable Echo facilities.

11. Is this a Teaching Hospital for Medical students/Residents

Recently saw a TV segment -in Karachi, Pakistan, a general hospital is equipped with modern Radiology facilities, mostly funded by local Philanthropists and is working out very well there.

Yes. B.J. Govt. Medical College is affiliated to Sassoon General Hospital. They have annual intake of 200 seats for MBBS and 8 seats for MD (Pediatric). In addition they have started PICU and NICU fellowships (affiliated to MUHS).
