PROJECT'S GOAL:

Feasibility study for acquisition and installation of air conditioning equipment and adaptation of the electrical infrastructure in the Maternity area of Hospital São João, in the county of Registro, aiming at humanization in the care of pregnant women and newborns.

PROJECT PERIOD:

The planning period, surveys, details, approval and formalization of the project is 01 (one) year.

The physical deployment deadline is estimated at 01(one) afterthe approval and release of the project's financial resources.

Total project implementation period will be 02 (two) years.

PROJECT DESCRIPTION:

The project will have as fundamental scope the acquisition and deployment of air conditioners, specified according to each installation environment, Maternity area of Hospital São João, in the city of Registro / SP.

The hospital area used for maternity has 38 beds, considering accommodation and wards, and of these, 23 beds do not have air conditioning or equivalent equipment.

The estimated number of monthly care, based on the historical year to 2018, is 108 parturients, of these, 82 were attended with normal birth delivery and 26 with cesarean section.

The air conditioner, through its programming, allows regulating the temperature of the space, either heating or cooling, as needed. Most of the equipment also allows regulating other issues such as environmental humidity and the renewal of the air through the function of ventilation.

This way, the air conditioning system during the summer will not leave the environment cold as it will also eliminate the humidity, while in winter will warm the environment and bring more moisture.

Finally, in both cold and hot weather, air can also be used to ventilate, filter and circulate ambient air.

TARGET AUDIENCE:

The Maternity Hospital of São João specifically serves the County of Registry, Sete Barras, Juquiá and Miracatu, being distributed as shown below, referring to the year 2018:

PROJECT PHASES:

Survey of the real needs for service.

Technical feasibility study in conjunction with the Beneficiary Entity, aimed at verifying the electrical conditions to support the new equipment.

Search for district and global partners for the project.

Establishment of an agreement with the entity would benefit for integrated action in the project.

Formalization of the Commitment Term for the use, operation and maintenance of the equipment with the Beneficiary Entity.

Acquisition of equipment.

Hiring a specialized company to install the equipment.

Installation of equipment.

Lecture Circuit for Hospital employees and pregnant women in the main cities served by the Hospital (Juquiá, Miracatu, Registro and Sete Barras).

Monitoring and evaluation of project results.

FOCUS AREA:

Maternal and Child Health, with emphasis on facilitating access to essential medical care and treatment for mothers and children, through the installation of environmental refrigeration equipment.

SUSTAINABILITY:

Considering the consequent rise in temperatures, and the environments of the maternity ward propagate high waves of heat, the proposed alternative is the one that best adapts, simply by maintaining the rigor in the care of use and maintenance.

The equipment to be donated will be of brands consolidated in the market for its quality and availability of spare parts. The Hospital São João will provide a preventive and corrective maintenance team, keeping the rigor in the care of use.

The partnership in the Project foresees the realization of lectures, educational campaigns and guidance to the public user of the equipment and satisfaction surveys for the implementation of the Project.

VISIBILITY:

The project's visibility plan for Rotary should be structured along with the project steps.

The National Health Surveillance Agency (ANVISA), through resolution No. 050 of February 21, 2002, provides on Technical Regulation for planning, programming, preparation and evaluation of physical projects of health care establishments:

7.5.1 - Conditioned air (AC)

The sectors with conditioning for comfort purposes, such as administrative rooms, internment rooms, etc., must be served by the basic design parameters defined in the ABNT NBR 6401 standard. The sectors intended for asepsis and comfort, such as operating rooms, ICU, nursery, parenteral nutrition, etc., must meet the requirements of NBR-7256. In the care of the above mentioned enclosures, due care must be taken, mainly because it involves works and treatments aimed at the analysis and eradication of infectious diseases, therefore, filtering systems, air exchange, etc. must be observed. The entire compartmentation of the EAS established by the architectural study, aiming to meet the safety of the EAS and, especially, avoid contact of patients with infectious diseases, must be respected when sectoring the air conditioning system.

Electrical outlets: The air outlets must not be close to the exhaust ducts of kitchens, toilets, laboratories, laundry rooms, fuel gas plants, generator sets, vacuum, internal parking and building, as well as other places where there is an emanation of infectious agents or harmful gases, establishing a minimum distance of 8.00 m from these places.

Air Renewal: The artificial air conditioning system requires forced air supply and exhaust, meeting the requirements regarding the location of ducts in relation to the fans, points of air exhaust and air intake. All air return must be done through ducts, being sealed the return through an open system (plenum). For the sectors that need constant air exchange, an energy system must be provided to meet the minimum conditions of use of the enclosure when the main electrical system is missing, with a minimum interruption period (see item 7.2.1.).

Noise level: The noise levels caused by the air conditioning, insufflation, exhaustion and diffusion system may not exceed those foreseen by the Brazilian standard NB-10 of ABNT for any frequencies or groups of audible frequencies.

Vibration: The air conditioning system may not cause, at any point in the hospital, mechanical vibrations of the floor or structure that impair the stability of the construction or the normal work of the EAS, obeying the compatible and specified criteria for each application.

MEASUREMENT

The main outcome measurement indicator for this project will be the rate of satisfaction of pregnant women and reduction of risks to the health of newborns.

In addition to this indicator, other results can be measured.

BUDGET:

Based on the goal of installing 23 units of air conditioners in the Maternity area of Hospital São João, the total estimated cost of the project will be US$141.250,15, according to the estimated budget of the table below.

ADDITIONAL INFORMATION:

HISTORY OF THE INSTITUTION: The Hospital São João is a philanthropic entity of medium complexity, located in the city of Registro/SP, maintained by the Association of Protection and Assistance to Maternity and Childhood Registration. (APAMIR), also non-profit, is a partner of the Unified Health System (SUS); it also plays an important and strategic role in the regional context, since it is the institution responsible for the second largest number of hospitalizations in the region.

HUMANIZED NORMAL BIRTH CENTER OF THE HOSPITAL SÃO JOÃO: The São João Hospital has innovated with several actions for excellence in the care of pregnant women and children, which differentiate the institution from any other maternity hospital in the region. Among these actions is the implementation of the Humanized Normal Birth Center (C.P.N.H.). The institution implemented the first C.P.N.H., a vanguard initiative in the Vale do Ribeira region. The C.P.N.H. is a safe, cosy and comfortable place, where humanized and quality assistance is offered to the mother and soon to the low-risk newborn (figure 1).

The Center has as a priority the humanization in the attendance. With this action, Hospital São João assumes the commitment to provide humanized assistance to mothers who want and can have normal birth from prenatal (figure 2), where they can count on environment/space. Adequately structured to provide pregnant women with the necessary warmth and ambience so that the normal birth can occur naturally and can count on the company of the relative or friend that the pregnant woman chooses. The Centre is included in the recommendations of the Ministry of Health and in the National Humanization Policy of the Unified Health System. (PNH/SUS) to make normal birth an increasingly natural and familiar experience, as well as in the effort to reduce the volume of unnecessary surgical deliveries. Another differential of C.P.N.H. is the presence of doulas. Doulas is a word of Greek origin that means "the one who serves the other woman". The doulas are employees who will provide care to pregnant women in the hospital, if there is no companion. They offer physical and emotional support to pregnant women during pre-partum and childbirth. The doulas, Neusa Baffile and Camila ZiliSelunk, will be available to pregnant women when necessary. In the C.P.N.H., which has six beds with bathroom (with accessibility for people with disabilities), air conditioning and a 24 hour team present, the birth will take place on site, in a space private, where the woman remains - if she wants to be accompanied - before and during the during childbirth. Throughout the process she is encouraged to walk, to practice exercises, and to take the position you consider most comfortable in labour: lying down, squatting, kneeling or in the bath. The Normal Birth Center team, formed by obstetricians, pediatricians, obstetric nurses, nurses, assistants and nursing technicians, will also seek to favor a better beginning of life for the newborn through the encouragement of breastfeeding, the presence of family members at the time of birth. In the newborn, still inside the C.P.N.H., the tests of the foot, ear, and ear are performed. (Figure 4 e 5).

Providing, in this way, more safety in the baby's health and strengthening the social safety net for the newborn and the rescue of the image of normal birth in a pleasant, safe and healthy way. In Hospital São João in 2018, an average of 82.0 normal births was performed against only 26.0 cesarean sections per month.

HISTORY OF THE CITY OF REGISTRO - DEMOGRAPHIC DATA:

Registro is a Brazilian conuty in the state of São Paulo. It is located in the mesoregion of São Paulo's South Coast, in the São Paulo portion of the Vale do Ribeira and is accessed through the Régis Bittencourt highway (BR-116). It is limited to the North by the Cities of Juquiá, to the South by Jacupiranga and Pariquera-Açu, to the East by Iguape and to the West by Eldorado and Sete Barras.

Registro has about 80 neighborhoods distributed in 716,331 square kilometers of area, with about 56 thousand inhabitants (estimated population data 2018 IBGE). The cultural and economic diversity and population concentration make Registro known as "Capital of the Ribeira Valley", or also as "Capital of Tea", in allusion to one of the main products exported by the Municipality, mainly until the mid-1990s.

The County is officially (according to Decree No. 50.652 of March 30, 2006) the landmark of Japanese colonization in the State of São Paulo because it would have been the first place to receive Japanese immigrants interested in investing in their own production in the State of São Paulo. The Iguape Group (colonies of Registro, Sete Barras and Katsura or Giporuva) was chronologically the first large Japanese colony in Brazil, and also the first among colonies founded by Japanese private capital.

The city is home to Japanese immigration cultural assets that will be preserved by the National Historical and Artistic Heritage Institute (Iphan): the headquarters of Kaigai Kogyo Kabushiki Kaisha, former tea factories and residences of the first Japanese settlers, the Anglican and San Francisco Xavier Episcopal Churches, and the first Assam varieties of tea seedlings. Sculptures by artist Yutaka Toyota made of material from the old tea factories, warehouses and rice processing mill are installed in various parts of the city of Registro.
