Proposal and Goals: Promote disease prevention programs, with the goal of limiting the spread of communicable diseases

Elements of sustainability:

a) Awareness of citizens and health technicians about disease treatment and prevention.

b) Providing technical equipments offering specific health support to neighboring cities and villages, increasing the number of services to those without medical support.

c) Providing technical platform and training in its operation for the tracking and monitoring of disease diagnosis and treatment;

1. INTRODUCTION

Rotary Club of Ribeirão Preto belongs to Rotary 4540 District of Rotary International, a region that host clubs in the north of the State of São Paulo and southern of the State of Minas Gerais in Brazil.

The project involves the entire regional community, since the partner: the Hospital of Cancer of Ribeirão Preto receives patients from a regional area with a population of about 1.3 million inhabitants. In particular, the hospital assists patients who cannot afford to pay for private medical care. The project is sustainable and results will be a great achievement to the local community.

About the disease:

Breast cancer, today, has become a public health problem. Both incidence rates and mortality rates have been steadily increasing in Brazil. According to INCA - National Institute and Cancer in 2018 is expected 59,700 new cases of breast cancer, which is the one that kills the most.

Unfortunately, in Brazil 60% of the cases are still diagnosed late (stages III and IV). This rate reaches 80% when we include stage II of the disease. Thus, unlike in developed countries, mortality rates have been increasing in our country, as well as the costs of treatment.

Treatment and compliance:

Early diagnosis, in turn, is still the greatest weapon to reduce mortality due to this disease, impacting on the reduction of costs. In developed countries this has been happening, thanks to the encouragement and investment in population screening programs through mammography, associated with clinical examination of the breasts, which have proven to reduce mortality rates by at least 30%

A major problem has been the lack of no information and awareness among population about the disease, associated with insufficient access to mammographic exams offered by the Unified Health System (SUS).

Early detection is one of the most efficient methods used to diagnose cancer. In the case of breast cancer it is known that 95% of the cases diagnosed at the beginning have a possibility of cure, and in the case of uterine cancer , the possible cure is about 100%.

With this background the project is aligned with Rotary's focus area of prevention and treatment of diseases, destined to low incoming women.

2. PARTNERSHIPS

THE HOSPITAL OF CANCER FROM RIBEIRAO PRETO will provide the truck, the medical/hospital facilities and health team as physicians, nurses, other examinations, and all equipment and materials and all needed for all women cancer prevention examination who are the object of the project.

ROTARY CLUB de Ribeirão Preto - DISTRITO 4540, the International partner and the Global Fund / The Rotary Foundation will participate in the project for the purchase of equipment and the installation of cancer prevention equipment at the truck.

3. PROPOSAL

Increase the number of cancer prevention and information driving the equipped trailer to towns and small towns in the region in order to carry out cancer screening in areas without access to this procedure.

4. MAIN GOALS:

Proceed about 3.000 cancer prevention examination per year

4.1 Specific Goals

Prevent of breast cancer in women and men and uterin cancer in women without access to medical centers

Disseminate information and carry out preventive exams of cancer

Identify and provide a chance of cure to early positive diagnoses of cancer with immediate treatment

Purchase cancer breast and uterin câncer prevention equipments

Adapt the truck and Install the equipments.

5. BENEFICIARIES:

- Low income population living in the rural areas (villages, towns, districts and neighborhoods away from the city centre), who are unable to go to the hospital on their own and who has lower level of awareness of cancer.

7. COOPERATION: Cancer Hospital of Ribeirão Preto

Founded in 1994, the Cancer Hospital of Ribeirão Preto is a philanthropic institution, maintained by the SOBECCan Foundation (Foundation for Research, Prevention and Cancer Assistance). Currently, about 400 patients are treated monthly, from the city of Ribeirão Preto and twenty-four cities in the region. The hospital, as a non-profit organization prioritizes people who do not have medical insurance. The hospital is located in the city of Ribeirão Preto in a 1.296,04 m2 of built area. It is currently recognized as a municipal, state and federal utility. It is duly registered in the Municipal Health Council and in the National Council of Social Assistance. It is supported by volunteering identified with the cause of the institution, a cadre of employees involved with the foundation and a clinical body dedicated to the patients who are treated here. 9. SUSTAINABILITY

9.1. Community needs

- Lack of cancer prevention test procedures.

- Lack of positive result treated reached to treatment

- Lack of departures of equipped truck.

9.2. COMMUNITY SURVEY FOR NEED IDENTIFICATION

The project team conducted a Community Survey to identify needs, using the tools: Community Meeting.

The Community Survey was conducted with 300 people receiving medical care at the Cancer Hospital. During the meeting participants were asked to express their opinion about the hospital. The results showed the good preparation of the hospital and satisfaction of the users. See enclosed (4).

10. PROJECT IMPLEMENTATION, TRAINING, COMMUNITY OUTREACH AND EDUCATIONAL PROGRAMS:

1. Purchase and installation of equipment at the truck

1.1. Departure of the truck to visit other cities and neighborhoods

The mammography device and the vehicle, equipped will be provided by the Rotary Club. On both sides of the vehicle, Rotary International logo and names of clubs that donate money, will be written.

To reach the villages and towns; the hospital team, the specialist doctor, nurse, technician and the driver who will drive the truck . The vehicle will park in different villages in order to test women invited with the announcement.

The test results will be evaluated by experts and delivered related departments. Furthermore, results which could indicate diseases will be delivered the nearest state hospital to make detailed examination and to begin treatment.

2. Community outreach procedure at the truck:

2.1 Pre- exam:

The patient is received by the receptionist, for registration.

A Nurse performs clinical screening, pressure and heart rate, and directs the patient to clinical care and accompanies the patient in the procedure.

2.2 Collect Papanicolau:

- Sending the blade to the Hospital

- The biomedical of the hospital does the cytopathological analysis of the blade

- Delivery of Diagnosis and referral to the doctor or to the local health post

- After-care will be done at the hospital or at the local hospital.

2.3 Mammography

Made by radiology technique

- The image of the exam is sent to the hospital

- The hospital radiologist examines and makes the report, and the positive cases will be referred for clinical follow-up and treatment in the hospital or at local municipality

2.4 Ultrasound

Medical transcriptionist and breast ultrasonographer

Made by the doctor in the truck, and delivery printed report on site

Education Program: The medical and care team will be trained and supervised for management of medical equipment and outpatient support.

SIGNIFICANCE TO ROTARY

The proposed project is in perfect synchrony with the Rotary focus area of, disease prevention and Treatment. The immense need will take it to the nook and corner of the district, significantly boosting the Public image of Rotary, and making it well known as a household organization. The project itself will have a high impact in the society in tune with the Rotary objectives.
