Proposal Form

Mata amardevi memorial Rotary Eye Hospital is being established at Chandan Nagar Raya about 20 kilometre from Jammu City , J & K ( India ) . The land for the Hospital has been donated by a Rotarian and the structure (Building) has been completed with the help of other Rotarians and the civil society of the Jammu who contributed both in kind and cash.

In order to commission the said eye hospital, certain hospital and Theatre equipments including instrument are required for which the matching grant is sought through this proposal as detailed at section "F Budget. "

In india , 80 % of the population lives in the Rural Area where adequate medical facilities do not exist particularly in the ophthalmology ( Eye )discipline with the result that about million people are blind from cataract alone which is otherwise treatable provided proper eye care centers are available nearby . This proposed hospital in the rural area is a step in the right direction to prevent the blindness and timely treatment to the beneficiaries for long term benefits.

The poor people in and around the Village Chandan Nagar ( Raya ) cannot afford the costly eye treatments in big cities resulting in numerous miseries and agonies they suffer in their lifetime.

This hospital being a Charitable institution shall be run on no profit / no loss basis so as to provide affordable eye treatment to the poor beneficiaries and in certain cases, free treatment shall also be provided to the people living below the poverty line,
