Your application has undergone an initial review and can not be funded until the next Rotary Year. Please submit the following information at your earliest convenience to complete your request:
• Project Implementation. Please confirm if the surgical center at Holy Innocents Children's Hospital where the equipment will be used has been built.
• Host Partner Participation. The host committee of RC Mbarara should describe how club members will participate in the project implementation. For example: How will communication be maintained with RC Fallbrook until the closure of the project? Will Rotarians be arranging for transportation of the equipment to the hospital or is this handled by Dunrich Cargo? Will Rotarians visit the hospital?
• Host Committee and Authorizations. The application does not include the signature of the third project contact listed on the host parter page, Rotarian Alex Koyekoma. Please provide Rotarian Koyekoma's signature on the authorization page (attached). If the third project contact is Rotarian Sam Muguta Mwesigye, who did sign the authorization page, please provide his contact information.
• Budget. Please provide a quote or invoice from International Aid if available.
